The Patient Care Coordinator works with Patient Care team, the Business Office, Medical Records, and the Clinical Management team to coordinate and assure updated patient information in the electronic medical record, copies of necessary forms, and assessment of services due to all patients. This role will include: contact with external facilities for necessary information, EMR chart updates, patient contact and follow up on services due, quality assurance duties for patient care protocols related to Amoskeag Health’s Quality Improvement Program, and other related duties.

Qualifications:

Education: High School Diploma or equivalent required. Completion of a Medical Assistant Program or equivalent training preferred.

Experience:

Experience with computers required
Prefer experience in an ambulatory health care environment.

Roles and Responsibilities:

The Patient Care Coordinator functions to assure that patients are coming into visits prepared for the visit purpose, to assure patients are getting necessary appointments based on their age, gender and disease condition following evidence-based protocols, and to assure patients are educated about and offered preventive care based on evidence-based protocols for their age, gender and disease condition. This individual will be responsible for:

Health Maintenance/ Chronic Disease Visits (Asthma, Diabetes & Hypertension):

-Scheduling overdue appointments including Preventive testing/services based on standing orders and outstanding referrals
-Emergency Department Follow-up Visits

-Checking to see if we have a copy of the Emergency visit discharge report scanned in patients Electronic Medical Record
-If not, requesting the information and tracking to assure report is available before the visit
-Schedule follow up appointment as directed by the provider
-Manchester Mental Health Consult chart update
-Pediatric to adult provider transition
-Responsible for Provider signature folder (populating as much of the form as possible prior to presenting it to the medical provider for signature)
-Cancer Screening outreach
-Complex appointment scheduling

Secondary Functions

-This individual may be asked to work on monthly reports for patients due for any QI measure by the QI Coordinator if time allows, and be asked to send a reminder letter to the patient to inform
This individual will be asked to assist with data gathering for various special projects, grants and other purposes.
-This individual may need to assist with data clean up or patient follow up on any of the issues listed above or for other new projects or problem areas identified.
-Assist patient in identifying and seeking help for specific problems. Provide education, within their scope, regarding health issues the client is involved with.  This will be under the direction of the medical providers
-Schedule additional laboratory tests and makes outside referral appointments (to the hospitals) with direction from the medical provider. Records pertinent data in the medical record and utilize an appropriate tracking system.
-Assist with various health screenings, to include but not limited to hypertensive/blood sugar screening.
