Then come join our Behavioral Health team in the Integration model at a Federal Qualified Health Care setting. Amoskeag Health offers Medication Assisted Treatment (MAT) for opiate use and alcohol misuse disorder. Amoskeag Health uses Suboxone and Vivitrol medications to support recovery.
The Substance Abuse Disorder and Medicated Assisted Treatment Counselor position consists of 2 main core functions including; providing evaluation and treatment of clients who may have a substance misuse condition and/or psychiatric condition utilizing personal interviews and formal evaluation tools. They will work with the behavioral health staff and primary care providers of the client to coordinate care. They may provide urgent interventions, evaluations and ongoing individual counseling and/or support groups. The supervision component of this position will involve supervision of the MAT Community Health Worker and may include supervision of new LADC/MLADC personnel in the future. This position requires the individual be able to work well in a fast paced environment, have the ability to work well independently as well as communicating effectively with the medical professionals. Also must have some ability to be flexible with their schedule.
Qualifications:
Education: Master’s Degree or higher in Social Work, Psychology, Mental Health Counseling or Marriage and Family Therapy required. State of NH Licensed Independent Clinical Social Worker (LISCW) or other related field required.
Experience:
MLADC Three to five years of experience with SUD preferred
Bilingual ability in English and Spanish preferred.
Two years’ experience in the healthcare field preferred
Roles and Responsibilities:
-Provide comprehensive assessment, evaluation, consultation, diagnosis and intervention of substance use and co-occurring disorders.
-Provide effective treatment planning and assisting clients in successfully achieving goals.
-Evaluate crisis situations and apply appropriate interventions.
-Establish and monitor group development for MAT patients.
-Supervise program staff, assisting with hiring, training, coaching, mentoring, and evaluation.
-Maintains a competent and engaged staff by communicating regularly, identifying training needs, completing performance appraisals and providing coaching.
-Provide financial oversight of the program, including monitoring the budget and grant requirements.
-Actively participate in meetings internally and externally that support Amoskeag Health’s MAT/SUD programming.
-Assist the primary care team and MAT team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
-Assist in the development of policies and procedures in regards to the SUD & MAT program.
-Monitor grant requirements and funding sources to promote and enhance SUD & MAT services.
-Maintain accurate, timely documentation in the client’s medical records of all client contacts, case planning and the client’s plan.
-Provide regular audits of staff’s documentation to promote learning in the behavioral health team and ensure policies and procedures are being followed.
-Facilitate the workflow of the MAT clinics, providing guidance and support as necessary to the MAT providers.
-Exhibits non-judgmental, empathetic and supportive approach when communicating with patients/families and staff.
