Bilingual/Bicultural Community Health Worker

PRIMARY DUTIES: The Community Health Worker (CHW) works with clients and families using an ecological approach to promote, maintain and improve health conditions through education, counseling and screening activities. The CHW will play an integral role interacting with diverse community groups to build trust, as well as establish and maintain community relations. The CHW will establish linkages within the community to increase access to testing and care services, support employment efforts and housing security as well as support case investigations, monitoring, and disease control. The CHW will support COVID-19 response efforts: participating in weekly testing clinics, setting up testing appointments, contact tracing follow-up, distribution of prevention and health promotion materials (ie. Face masks) and; providing support to families who are placed on quarantine and/or isolation. The CHW will work across DPHCS programs with the Communications Specialist to develop health promotion and prevention messages in multiple languages and distribute information to businesses and food establishments. Position is full time, Monday through Friday 8 AM to 5 PM. Hourly rate not to exceed $23.2452. *Position is grant-funded.

QUALIFICATIONS: High School Graduate, GED or College Degree (2 years): Community Health Education, social work or related field. 3-5 years relevant job experience or related work experience as a paraprofessional in health care or medical assistant. Public Health Experience Preferred. Possess computer literacy skills to perform the requirements of this position. Knowledge, skill or training in phlebotomy, medical interpretation, and social support services. Bilingual skills required (English/Spanish; English/Portuguese; English/Swahili).
