Job Summary
The Adult Case Manager is responsible for the coordination, oversight, and monitoring of services tailored to best address clients’ strengths, needs, interests, and preferences. Ongoing case management requires routine, planned contact with the individual, family, and those responsible for implementing the service agreement in efforts to assess progress toward goals and ensure overall health and safety. This Case Manager will work with adults over the age of 21 who experience a Developmental Disability or Acquired Brain Disorder.
Essential Duties & Responsibilities
Assist with coordination, oversight and monitoring of client through the continuum of care as indicated by their annual service agreement.
Address needs as they arise and seek out appropriate resources and referrals.
To meet with clients in their home and in the community as required by regulations.
Coordinates supports and services with Area Agencies and other vendors/service providers ongoing.
Contact clients, family, guardians and service providers ongoing to assist development, implementation and progress with goals and services specific to the Client’s needs.
Review client progress towards goals and objectives.
Maintain records and files, and prepare clear, concise, and timely documentation.
Identify, network, and establish contact with key stakeholders needed by the client to include but not limited to Area Agencies, Guardianship Agencies, local support services, Medicaid Office, etc.
Education & Experience Requirements
Bachelor’s Degree in a related field is required.
Experience with Development Disabilities, Acquired Brain Disorders, mental health &/or medically complex individuals as well as Medicaid, Medicare is a plus.
Demonstrated competencies in the following areas: creative problem-solving skills, strong teaching skills, leadership skills, visionary ability, opportunistic nature, professionalism, sensitivity to confidentiality, and familiarity with relevant compliance rules and regulations.
Knowledge, Skills, and Abilities:
Ability to use a computer; expertise in Word and Excel; familiarity with other Microsoft Office products.
Excellent verbal and written communication skills.
Strong interpersonal and organizational skills.
Hold a valid Driver’s License and maintain a clean driving record.
