**SIGN ON BONUS $500**

Children and Youth Case Manager is responsible for coordination, provision, and monitoring of services tailored to best address clients’ strengths and needs. Ongoing case management requires frequent, planned contact with the family to assess progress toward goals.

LOCATION – Manchester, NH


Assist with the movement of the client through the continuum of care as indicated by the treatment plan and treatment team.
To meet with clients in their homes and provide structured family interventions as needed.
Coordinates with other agencies and groups to provide plans and services to clients.
Contact clients and/or family after they have been identified; assist the Clinician in developing treatment plans that are specific to their needs.
Review client progress towards goals and objectives.
Provides crisis intervention with guidance.
Maintain records and files, and prepare clear, concise, and timely reports.
To identify, network, and establish contact with collateral services needed by the client.
Meet with assigned clients and their families as frequently as outlined in the treatment plan, and provide home visits as required in the licensing agreement.
Maintain monthly contact with the referring agency and provide required progress reports.


BA/BS in a related field is required.
Experience with medically complex individuals is a plus.
Demonstrated competencies in the following areas: creative problem-solving skills, strong teaching skills, leadership skills, visionary ability, opportunistic nature, and familiarity with relevant compliance rules and regulations.


Ability to use a computer; expertise in Word and Excel; familiarity with other Microsoft Office products.
Excellent verbal and written communication skills.
Strong interpersonal and organizational skills.
Actively involved with sharing ideas and creative solutions.                                                                     Hold a valid Driver’s License and maintain a clean driving record.
